A work injury is not a diagnosis. It tells me where or how the problem happened, not what was actually injured. I am Dr. Brian Donovan, and at Nuro Chiropractic in Brooklyn that distinction shapes the entire first visit.

Two Very Different Kinds of Work Injury

The Single Moment

I lifted something heavy and felt my back go. I slipped and landed on my shoulder. I twisted my knee carrying something. These arrive with a clear before and after.

The Slow Build

Eight hours a day at a computer turning into persistent neck and upper-back pain. A construction worker developing shoulder or elbow pain after months of repetitive lifting. A healthcare worker with low back pain from transferring patients. Someone on their feet all day developing knee, foot, or tendon problems.

Both are work injuries. They need different questions, and often different plans.

How I Evaluate a Work Injury at Our Brooklyn Office

  • 1 Establish exactly what happened: one accident or a gradual build, what you were lifting, pushing, pulling, carrying or reaching for, what position you were in, and when symptoms began.
  • 2 Find out what has changed functionally. Can you bend and lift, sit at a desk, stand through a shift, reach overhead, grip a tool, climb stairs, or safely do what your job requires.
  • 3 Examine the involved region. For a low back that means lumbar range of motion, strength, sensation, reflexes, orthopedic and neurological testing, movement patterns, and the hip. For an extremity, range of motion, strength, stability, and the relevant tendons.
  • 4 Add a full neurological examination whenever there is numbness, tingling, radiating pain, or weakness, since a nerve root or peripheral nerve may be involved.

Imaging is not automatic just because an injury happened at work. Digital X-ray is here when appropriate. MRI, diagnostic ultrasound, or nerve testing comes in when I am concerned about a disc, a significant tendon or ligament injury, or nerve compression.

Treatment Follows the Injury, Not the Paperwork

  • Mild mechanical back or neck injury: appropriately selected chiropractic adjustments, mobility and stretching, massage and soft-tissue treatment, IASTM, and progressive exercise.
  • Acute and guarded: a gentler start with comfortable movement, soft-tissue work, mobility, or low-force technique, progressing as tolerance improves.
  • Disc-related conditions: spinal decompression when clinically appropriate. Chronic tendon problems at the elbow, shoulder, knee, Achilles, or plantar fascia: SoftWave therapy.
Comfortable on the Table Is Not the Goal

If your job requires lifting forty pounds repeatedly, eventually we have to find out whether your body tolerates lifting again. If you work overhead, we rebuild the strength and endurance to work overhead. If you sit all day, we deal with mobility, strength, ergonomics, and your tolerance for prolonged sitting. Returning to work safely involves more than getting a pain score down.

Documentation and Workers Compensation

We keep a clear record of how the injury occurred, your symptoms, objective examination findings, diagnosis, functional limitations, treatment, and progress over time. When appropriate we coordinate within the New York Workers Compensation system and with the other providers on the case.

When a Work Injury Needs Medical Care

Significant trauma with obvious deformity, suspected fracture or dislocation, inability to bear weight, major swelling, or uncontrolled bleeding needs emergency evaluation. Head injuries deserve particular attention: loss of consciousness, repeated vomiting, confusion, seizure, worsening severe headache, or significant balance problems need prompt medical assessment.

New bowel or bladder dysfunction, numbness in the saddle or groin region, or rapidly progressive leg weakness requires emergency assessment. Progressive weakness, worsening numbness, loss of coordination, or difficulty walking may call for MRI, nerve testing, or a specialist.

I co-manage without hesitation. An orthopedist for a significant shoulder or knee injury, a neurologist for neurological symptoms, a pain-management physician for certain spinal conditions. Referral does not mean conservative treatment failed. It means you need something outside what I should manage alone.

Work Injury FAQs

Do you accept workers compensation in Brooklyn?

Yes, we take workers compensation and no-fault. Documentation is built into the care rather than added afterwards, so the record of your findings and progress is there when the case needs it. We can also coordinate with the other providers involved.

My injury built up over months. Does that still count?

Plenty of genuine work injuries have no single dramatic moment. Repetitive lifting, sustained postures, and prolonged standing all produce real problems over time. Clinically it changes the questions I ask rather than whether I take the complaint seriously.

When can I go back to full duty?

When your body tolerates the actual demands of the job, which is a different question from whether you are comfortable resting. I would rather build up to your real lifting, reaching, or standing requirements and know you can handle them than sign off early and see you back in six weeks.

Should I keep working while I recover?

It depends on the injury and what your job asks of you. Modified duty often works better than complete time off, because staying somewhat active usually supports recovery. Some injuries genuinely need a period away, and that is a case-by-case judgment rather than a policy.

Do I need my employer’s approval to come in?

Call us and we will walk you through how it works for your situation, since the process varies. What matters clinically is getting examined rather than waiting weeks while you guard the area and develop a second problem on top of the first.
